Description

Bridges helps patients and their carers to make links with and access services already available. When appropriate, we also provide additional support to maximise people's quality of life at home. Bridges supports patients and carers within Birmingham and Solihull who are at an end-of-life palliative care stage.

We provide:

• A home visit to understand your needs
• Co-ordination of support
• Help with domestic tasks in the home
• Someone to sit with the patient so that the carer can have a break
• Someone to talk to who listens
• Help to find reliable informationHelp to access benefits and manage your finances
• Help to find spiritual support

Monday to Thursday 9am-5pm and Fridays 9am-4.30pm.  The service is free of charge.
